Please Describe Your Project In Detail | To disseminate information pertaining to COVID for prevention and offer a site for screening and testing. My facility offers 2 suites in the center of District 9 in East Chattanooga on Glass Street. East Chattanooga has one of the highest population of African Americans, highest rate of poverty, highest crime rate, homicide rate, poorest health amongst African Americans, and the highest incidence of COVID 19! District 9 My program will include: A. Media updates via print and social media about the services available ( ie. screening/testing/vaccinations) B. Monthly Webinar to discuss safety and the most recent updates concerning the latest news and recommendations from CDC and the Government). Interactive discussions involving Q & A. Experts in the health profession will be serving as panelists C. Ongoing prevention program involving free testing, and vaccinations D. Counselors to provide consultations to individuals needing assistance with depression and other assistance for financial help related to loss of income due to COVID E. Webinars to address violent crimes issues in our community hosted by law enforcement agencies F. Partnerships with Women Health, Health Department, Police Department, and the City of Chattanooga and other resources that can be made available for individuals needing assistance related to COVID 19 (job placement, rental assistance, counseling services) |

American Rescue Plan is designed to: a. provide funds for vaccinations, b. provide immediate, direct relief to families bearing the brunt of the COVID-19 Crisis, and c. to support struggling communities. My program provides services to address all 3 areas and categories(A, B, C, D, E, and F of the project detail) |

What milestones would you use to measure your project’s progress? | Every person receiving services will be tracked and logged into our system: 1. Name, ethnic group and age address, services received, and outcome of service(follow up survey) 2. Data on COVID (positive results vs. negative results tracked by zipcode) 3. Number of participants receiving assistance for employment, bills, etc. as a direct impact from COVID related illnesses 4. Crime statistics reports quarterly for East Chattanooga |
How would you ensure accountability and transparency throughout the project lifecycle? | Monthly tracking reports and book keeping provided monthly, Committee consisting of Medical provider, community residents, affiliated partners including a representative from the Hamilton County Health Department and a representative from the City of Chattanooga and neighboring businesses |
If successful, how would your project benefit the community? | Our goal is to see a Reduction in the spread of COVID through education, prevention, vaccinations and by providing resources that will help those affected by COVID get the help they neede via resources we will provide through our affiliated partners. We will see a decline in COVID, crime, depression, death and displacement of individuals from their homes due to loss of income. Individuals that cannot find the resources can utilize our facility to navigate through the system and get help as a one stop |
How will you attract community buy-in for your project? | We will involve business owners and community residents (neighborhood association) in the process and have monthly meetings to update everyone about the progress of the project and involve them in the the project (passing out literature, phone calls, virtual neighborhood meetings etc., Radio and TV advertisement) |

Is there anything else you would like us to know about your project? | This project will have a profound effect on helping a community in the greatest need. We have a mixture of health professionals and counselors to provide the care and services needed to assist individuals affected the most. Our COVID relief center is located in an economically depressed community, with the greatest incidences of COVID and the highest levels of depression and violent crimes in Chattanooga (East Chattanooga) |
